- Past history of any other diseases
- Family history of any diseases
- Appetite, thirst
- Sleep & Dreams
- Desires and aversions to specific food & drinks
- Urine, stool, sweat
- Sexual complaints if any
- Menstrual and Obstetric details
- Mind symptoms (your emotional state, basic mental nature, any stress etc.)
- Copy of laboratory investigations done with reports
- Any other associated complaints. Please attach photographs in all relevant cases.
We will contact you if more information is required.
